The Humber River Hospital’s former Church Street site was renovated and reopened after closure as a Reactivation Care Center due to an increasing local need for alternate level of care beds in the region’s hospitals. This new center allows care to be provided in a more appropriate setting to less acute patients and eases hospital capacity challenges in the region.

After completing the successful relocation of the Humber River Regional Hospital to their new 1.8 million square-foot facility in 2015, and executing a similar mandate into Humber’s former Finch Site, HCR was sought after to provide Transition Project Management, Operational Readiness Review, Facility Activation Planning, and Asset and Patient Relocation services in support of the reopening.

HCR developed an effective project management organizational structure which supported all aspects of the transition. Following this, an Operational Readiness Review and Facility Activation Planning was completed which assessed the status of operational and building readiness and provided guidance and support to ensure that all programs and services at the Church Site were ready to begin immediate, organized, safe and efficient operations on the first day of occupancy.

HCR also planned and coordinated the multi-phase relocation of all patients moving into the newly renovated facility. The first phase of patient moves was completed in December 2018 with the relocation of ninety-four patients from the Sunnybrook Bayview Campus, Sunnybrook Holland Campus and Humber River Hospital, marking the official opening of the Reactivation Care Center Church Site.

HCR returned to provide the same services for the second and third phases of patient moves. The second phase was completed in May 2019 and saw a total of 88 patients safely moved from six separate off-site locations (Credit Valley Hospital, Mississauga Hospital, Queensway Health Centre, Southlake Regional Health Centre, St. Joseph’s Health Centre and Brampton Civic Hospital) in a single day. The third phase was completed in January 2020 and saw the single day move of 15 patients from 3 separate off-site locations (Credit Valley Hospital, Mississauga Hospital and Queensway Health Centre).
