Health Care Relocations provided Health PEI transition planning and relocations services with post move consolidation activities. This included the resident transfer from the existing facility to the new location approximately 2 miles away. The movement of residents took place on day 4 of the relocation culminating in the movement of 82 residents into their new “households” which is part of the new model of care introduced by Health PEI for long term care occupants.
